La obesidad en el embarazo es una condición frecuente, al igual que en la población general. Según la Organización Mundial de la Salud (OMS), es un problema que afecta casi a 302 millones de personas en el mundo.

Por tanto, son muchas las mujeres que comienzan la gestación con un peso mayor del deseado. Lo que sucede es que la obesidad en el embarazo altera no solo la salud de la madre, sino que puede causar complicaciones en el bebé y en su desarrollo. Por eso te explicamos todo lo que debes saber sobre el tema.

¿Qué se considera obesidad en el embarazo?

La obesidad en el embarazo, al igual que en cualquier momento de la vida de una mujer, se define como un exceso de grasa corporal. De manera general, para clasificar los problemas de sobrepeso y obesidad se calcula el índice de masa corporal (IMC).

El índice de masa corporal se calcula dividiendo el peso por la altura elevada al cuadrado. De esta forma, cuando el IMC es superior a 30, ya se considera que una persona sufre obesidad. Se distinguen varios grados en función de este dato:

  • Obesidad de clase I: cuando el IMC está entre 30 y 34,9.
  • Clase II: cuando el índice de masa corporal se encuentra entre 35 y 39,9.
  • Obesidad extrema o de clase III: el valor supera el 40.

Es importante saber que este parámetro es, en realidad, una medida de aproximación. En algunos casos, como en personas que tienen mucha masa muscular, el IMC puede ser alto, aunque no sea a expensas de la grasa. Por eso solo se debe utilizar de manera orientativa.

El problema es que la obesidad tiene un gran impacto en todos los ámbitos de la salud. Aumenta el riesgo cardiovascular y, además, perjudica la fertilidad.

Según un estudio publicado en la Revista Peruana de Ginecología y Obstetricia, esta condición produce cambios hormonales que acaban disminuyendo la fertilidad femenina. De hecho, no solo altera la fertilidad por métodos naturales, sino que puede llegar a reducir el éxito de técnicas como la fecundación in vitro.

¿Cómo puede afectar la obesidad en el embarazo?

La obesidad en el embarazo es un factor de riesgo tanto para la madre como para el feto. En lo que se refiere al proceso de la gestación, se ha visto que esta condición aumenta de manera notable la posibilidad de sufrir complicaciones.

En primer lugar, como explica la Clínica Mayo, se aumenta el riesgo de abortos espontáneos. Esto consiste en que el bebé muere dentro del útero durante las primeras 20 semanas de gestación. Del mismo modo, se ha vinculado con abortos recurrentes y el nacimiento de fetos muertos.

Al dar a luz, la obesidad en el embarazo hace más frecuente la realización de cesareas. Esto, a su vez, implica una recuperación postparto más lenta e infecciones de la herida.

La preeclampsia es una de las complicaciones más graves durante la gestación que tiene una fuerte asociación con la obesidad. Aumenta la presión arterial de la madre y puede dañar la placenta, comprometiendo el flujo sanguíneo que llega al feto.

Riesgos sobre la salud de la madre

Para la mujer gestante, la obesidad en el embarazo supone un riesgo muy importante para su salud. Según el estudio Dorica, la obesidad incrementa la posibilidad de sufrir un evento cardiovascular. Por ejemplo, se relaciona con la aterosclerosis y la enfermedad coronaria.

Del mismo modo, aumenta la presión arterial y el riesgo de padecer diabetes gestacional. Este trastorno consiste en el aumento de los valores de glucemia de la madre durante el embarazo. Puede provocar que el feto crezca más de lo normal (condición que se llama macrosomía) y que presente problemas relacionados con la hipoglucemia al nacer.

Riesgos de la obesidad en el embarazo sobre el bebé

La obesidad en el embarazo, como hemos señalado en el apartado anterior, puede provocar que el bebé tenga un tamaño mayor de lo normal. Esto no solo complica el parto, sino que hace que el niño sea más proclive a sufrir obesidad infantil y diabetes a lo largo de su vida.

Una investigación realizada por el National Institute of Child Health encontró que la obesidad en el embarazo puede provocar defectos en el tubo neural del bebé. El tubo neural es la estructura por la que se forman el cerebro y la espina dorsal. Por otra parte, también se ha asociado esta situación con alteraciones cardíacas y lesiones durante el parto.

¿Cuánto debe aumentar el peso en el embarazo?

Lo cierto es que el peso durante el embarazo suele aumentar y así debe ser. Los médicos realizan una estimación de cuánto se debe incrementar en límites saludables según cuál sea la situación basal de la madre. Es decir, antes del embarazo, en función del IMC materno, se recomiendan los kilogramos que se pueden coger.

En el caso de que ya haya obesidad y el embarazo sea de un solo bebé, se suele indicar que la ganancia de peso se limite a los 9 kilogramos. Si el embarazo es múltiple se pueden llegar a coger entre 11 y 19 kilogramos.

Esto se debe a que el intentar bajar de peso en esta etapa puede causar ansiedad en la madre. Incluso, conlleva un parto prematuro o que el bebé sea más pequeño de lo recomendado al nacer. No obstante, cada caso debe ser revisado de manera específica por el especialista.

Consejos para tener un embarazo saludable

Para poder tener un embarazo saludable, lo mejor es que sea planificado. De esta manera se pueden adoptar hábitos de vida especiales. Por ejemplo, se puede intentar bajar de peso para reducir la incidencia de la obesidad en el embarazo.

Lo ideal es mantener una dieta sana y equilibrada. Se debe reducir el consumo de alimentos procesados y ricos en grasas, al igual que eliminar la ingesta de alcohol o cualquier otra droga. Además, se recomienda introducir alimentos ricos en ácido fólico y hierro.

Del mismo modo, es importante tratar de hacer actividad física moderada. Si se coge un buen hábito de ejercicio, se puede seguir realizando durante el embarazo, lo que traerá numerosos beneficios para el bebé y para la madre. Por ejemplo, salir a caminar o nadar.
