Infecciones por Escherichia coli: ¿cómo reconocerlas?

La bacteria E. coli puede generar enfermedades muy conocidas, como la diarrea del viajero. Si bien se puede manifestar de otras muchas formas, la infección urinaria y la intoxicación alimentaria son las más frecuentes formas patológicas.

Las infecciones por Escherichia coli son la causa más común del síndrome conocido como diarrea del viajero. Más allá de esto, las diarreas son un problema grave en los países de ingresos bajos, pues según la Organización Mundial de la Salud (OMS), más de 500 000 niños mueren de forma anual por este padecimiento.

A una escala más global, esta misma fuente estima que 1 de cada 10 personas en el mundo se ve afectada en cualquier momento dado por una enfermedad de transmisión alimentaria (ETA), lo que se traduce en la pérdida de 33 millones de años de vida saludables a nivel global. Desde luego, estamos ante un tema más espinoso de lo que en un principio parece.

Por todos estos motivos, te presentamos las características de las infecciones por Escherichia coli —la bacteria entérica por excelencia— y cómo reconocerlas.

¿Qué es la Escherichia coli?

Antes de describir la patología en sí misma, tenemos que conocer al agente causal. E. coli es una bacteria que forma parte de la familia de las enterobacterias. Esta agrupación contiene más de 30 géneros y 100 especies de microorganismos gramos negativos, los que forman parte de la microbiota intestinal del ser humano y otros animales.

Se trata de una bacteria anaerobia facultativa —que puede desarrollarse en presencia o ausencia de oxígeno— comensal, es decir, que no genera perjuicios en el hospedador. Por desgracia, a lo largo de se evolución han aparecido cepas patógenas que producen diversos cuadros clínicos.

¿Cuáles son las infecciones causadas por Escherichia coli?

Según el portal profesional MSDmanuals, las distintas cepas de E. coli pueden causar infecciones urinarias, entéricas, invasivas o en otros lugares. Te las presentamos en las siguientes líneas.

Infección entérica

La bacteria que hoy investigamos se encuentra en el intestino de las personas. Sin embargo, algunas cepas han adquirido genes mutágenos y pueden generar cuadros de diarrea. Entre ellas encontramos las siguientes:

  • Enterohemorrágicas: estas cepas producen citotoxinas, neurotoxinas y enterotoxinas que causan diarreas sanguinolentas en el paciente. Suelen transmitirse por el consumo de carne mal cocinada.
  • Enterotoxigénicas: causan diarrea acuosa. Son las responsables de la diarrea del viajero.
  • Enteroagresivas, entereropatogénicas y enteroagregativas: también producen cuadros de diarrea acuosa, más o menos persistentes en el tiempo, según la cepa analizada.

Infección de orina

Según el portal Kidshealth.org, las bacterias son las causantes más comunes de infecciones en el tracto urinario. Entre los síntomas más comunes de estas infecciones de orina encontramos los siguientes:

  • Polaquiuria: necesidad de orinar de forma frecuente en pequeñas cantidades.
  • Ardor y picor: durante la micción.
  • Dolor: en la parte baja del abdomen.
  • Cambio de color en la orina: puede aparecer sanguinolenta.
  • Signos generales: fiebre leve, sensación de temblor y fatiga.

Infección invasiva

Según la Sociedad Española de Urgencias de Pediatría, casi el 15 % de las infecciones bacterianas invasivas se deben a E. coli durante la edad pediátrica. En este tipo de afección el cuadro clínico se vuelve más grave, pues la bacteria puede migrar a otros órganos y generar perjuicios en múltiples lugares a la vez.

Esto se puede traducir en un cuadro inflamatorio difuso, necrosis epitelial y ulceración de la mucosa gástrica en el paciente.

¿Cómo prevenir estas infecciones por Escherichia coli?

Los Centros para el Control y la Prevención de Enfermedades (CDC) nos muestran consejos para evitar cualquier tipo de infección por E. coli, sobre todo en el terreno alimentario. Algunos de ellos son los siguientes:

  • Lavarse bien las manos: con jabón, antes de cualquier tipo de contacto con la boca.
  • Comidas: limpiar, separar, cocinar y enfriar cualquier alimento que se vaya a consumir. No consumir masas o mezclas crudas.
  • Agua: no beber la que no tiene origen seguro.
  • Pasteurización: beber leche y zumos pasteurizados.
  • Viajes: extremar precauciones durante visitas al extranjero, sobre todo en zonas con circulación certificada de la bacteria y si la gastronomía no cumple con estándares básicos de higiene.

El cuidado en los alimentos es la mejor prevención

La infección urinaria es la manifestación más común de E. coli, seguida por ciertas cepas que generan cuadros gastrointestinales característicos. En países con una infraestructura sanitaria adecuada estos episodios patológicos se resuelven de forma rápida, pero hay regiones geográficas en las que la mortalidad es muy elevada por causas evitables.

Por ello, la mejor cura radica en la prevención: higienizar al máximo materiales de cocina, tratar todos los alimentos, evitar la ingesta de agua no clorada y lavarse las manos antes de cualquier contacto son las mejores barreras de contención ante las cepas patógenas de este microorganismo.
